All applicants are expected to complete this part of the application form.

Any fields containing the 'Public' Public Content Logo symbol, will be visible to the public.  These fields are there to help sell your organisation to other organisations who may be looking to use your services.  Please take care in entering appropriate and valid information.

All questions must be answered.  You will not be able to proceed to the next page until you have answered all the questions on the current page.  If you decide to change an answer on a previous page, you will be able to return to that page by clicking the 'Previous Page' button at the bottom of the form.

In completing the following questions please refer to the eligibility criteria section of the Membership Prospectus.  Your responses will be treated in confidence.  Greater Together will ensure the safe and secure storage of all application documentation and the information you submit will be used solely for the purpose of assessing your application for consortium membership.
